Sepsis-Induced Coagulopathy: A Comprehensive Narrative Review of Pathophysiology, Clinical Presentation, Diagnosis, and Management Strategies

Coagulopathy is defined as imbalanced hemostasis leading to bleeding or thrombotic events and is a major cause of mortality in critical illnesses such as sepsis. This month’s featured article is a narrative review of sepsis-induced coagulopathy (SIC) that highlights dysregulated crosstalk between the hemostatic and immune systems and details the pathogenic mediators that may lead to aberrant coagulation responses in sepsis. As depicted in the cover art, specific host- and pathogen-derived danger molecules released during sepsis interact with the innate immune Toll-like receptors in immune cells and platelets triggering thrombin generation with microvascular clot formation. This review sifts through both preclinical and clinical studies to examine the role of specific danger signals in development of coagulopathy, the impact on clinical presentation, and major clinical trials assessing therapeutic efficacy of various treatment strategies in SIC. A better understanding of the underlying molecular mechanisms and role of specific factors in SIC may advance diagnostic and therapeutic options.

IARS Leaders Connected with Global Colleagues at the 18th World Congress of Anaesthesiologists, March 3-7, 2024, in Singapore!

The International Anesthesia Research Society (IARS) leaders were thrilled to be able to connect with the global anesthesia leaders while attending the 18th World Congress of Anaesthesiologists (WCA2024), March 3-7, 2024, in Singapore. The IARS joined 40 other international organizations in the Global Anaesthesia Village at WCA2024 to exchange ideas and develop opportunities to collaborate together in the future.

IARS Board Chair Beverley A. Orser, MD, PhD, FRCPC, FRSC, Board Vice Chair Y.S. Prakash, MD, PhD, and Physician Executive Santhanam Suresh, MD, MBA, FASA, FAAP enjoyed connecting with leaders from global societies in the Global Anesthesia Village. Additionally, Anesthesia & Analgesia Editor-in-Chief Jaideep J. Pandit, MA, BMBCh, DPhil, FRCA, FFPMRCA, DM, MBA, also attended WCA2024 and was able to connect with global colleagues during an A&A Editors Meet-and-Greet session in the exhibit hall.

Applications are now being accepted for the 2024 IARS Mentored Research Awards (IMRA) through April 30, 2024! For more than 100 years, the core mission of the International Anesthesia Research Society (IARS) has been to encourage, stimulate, and support anesthesia-related research projects that will improve patient care and advance the specialty. IARS established its Mentored Research Awards to help create future leaders and prepare applicants to obtain independent research funding. Up to four research projects will be selected in 2024, each with a maximum award of $175,000 payable over two years. IMRA applications may be in any area of investigation (clinical, translational, basic science), but must have ultimate relevance to the broad practice of anesthesiology and its subspecialties.

With the support of a 2019 IARS Mentored Research Award, John Whittle, MD, was able to combine his undergraduate training in applied physiology with anesthesia research to explore strategies to prevent patient complications after surgery, expanding knowledge and the understanding of the benefits of prehabilitation. His IMRA-funded project, “Preoperative Exercise to Improve Vagal Tone (PREVENT) Study,” initially sought to determine if a high intensity interval training program could improve vagal tone and laboratory markers of inflammation and mitochondrial capacity. When the COVID-19 pandemic threw a wrench in that plan, Dr. Whittle, an Assistant Professor in Anesthesiology at Duke University Medical School at the time, pivoted and designed a study looking at longitudinal metabolism in patients admitted to critical care with COVID. The techniques uncovered in that study have now been applied to a new longitudinal metabolic study in surgical patients, which is currently underway. Today, this initial investigation has spurred even more progress for the study of prehabilitation. Now an Associate Professor in Perioperative Medicine at University College London, Dr. Whittle has set up a nutritional and physical prehabilitation program at the university which is linked to other national programs. He hopes this program will just be the start of developing patient-centered treatment strategies to reduce the risk of undergoing surgery. In an interview, he discusses how the IMRA helped him progress as an early-career researcher, his dedication to finding strategies to prevent complications after surgery and his hopes for the future of anesthesia research.
